Nature of notification

  1. PUBLIC - when the reporter agrees to full disclosure of their identity to both those involved in investigating the report and to third parties.
  2. CONFIDENTIAL - when the reporter's data is protected from unauthorized access, in particular by keeping the reporter's data confidential.
  3. ANONYMOUS - not allowing the reporter to be identified in any way.

What you can include in your application

  • Details of the reporting person (name and surname, place of work, position), including contact details (excluding anonymous reports)),
  • Date and place of the report,
  • Details of the person(s) who committed the violations that are the subject of the report (name and surname, place of work, position)
  • Description of the violations, including the circumstances in which they occured and the dates,
  • Description of the actual and potential consequesnces of these violations,
  • Actions taken by the reporting person to eliminate the violations or their effects (including previous reports, e.g., to superiors), if any,
  • Evidence collected by the reporting person to confirm the facts, e.g., documents, witnesses, if any.
